1993 Toronto Blue Jays.

The Jays started 1993 wearing the white front panel hats as their home hats.  Here admiring their 1992 championship rings at home earlier in the season:

You may remember during the season, the white front panel hat was retired.  They started wearing the solid blue road hat regularly as the home hat as well.  As seen in the 1993 World Series:

The 1978 Jets wore their Namath era jerseys in preseason with the (then) new helmet and logo.

This one is along similar lines.  The 2009 Saskatchewan Roughriders wore silver pants at home and black facemasks.  Minor uniform change in 2010.  Switched to white pants at home and white facemasks.

 

During the 2010 preseason, they had the white masks on but were still wearing the silver pants:

Another hockey one just came to mind.

 

1967-68 NHL Seals.  Started the season as the California Seals.

Renamed themselves the Oakland Seals in December 1967.  The logo changed from using a "C" to an "O".  The numbers on the back of the green jerseys were changed due to fan complaints about visibility.

I do remember this, but I'm guessing we won't see this happen anymore.  The fact that new logos/unis are leaked so far in advance now and merchandise is so readily available for purchase online makes a uni change a little less special than it used to be.  Back in the '90s I was still ordering stuff out of catalogs and a lot of the time if a team made a change, the new merchandise wasn't even available until the season was already underway.
