I'm assuming just a single front view of the jersey is all we are allowed to have for the jersey. No side or back views or anything.

You can have a back view to show the numbers on back. No side view is necessary - I don't think I've ever seen a hockey template with a side view.

Before I go sending this off here, does the primary logo only belong on the jersey itself or should the primary logo be shown enlarged by itself and also on the jersey?

You can either present just the jersey (and socks), as long as the primary logo is new, or both the jersey and an enlarged version of the primary besides the jersey.

You will not be disqualified for submitting only the jersey if a new logo is on it. But you might benefit from showing the logo by itself and enlarged to show the detail.
