They don't make every size - they skip a couple.

Let's take a look at the jersey I bought a couple weeks ago. It comes in:

Now, I usually wear a 42, so I bought the 44. It's pretty tight for a 44, but it works well on me. So I'd say go up to the next size, and you'll probably be alright.

For player issued jerseys, they make them in the in between sizes, like 42, 46, 50, etc. You can sometimes find some that were made for players but never quite made it there on eBay. I've got a Devil Rays road 42.
