The Chicago Cubs changed their uniforms in both 1907 and 1908, before their World Series championship wins in each season (and to date, their only two championship wins).

From this, in 1906 (as the Chicago Orphans): http://exhibits.baseballhalloffame.org/dressed_to_the_nines/detail_page.asp?fileName=nl_1906_chicago.gif&Entryid=92

To this, in 1907: http://exhibits.baseballhalloffame.org/dressed_to_the_nines/detail_page.asp?fileName=nl_1907_chicago.gif&Entryid=108

And finally this, in 1908: http://exhibits.baseballhalloffame.org/dressed_to_the_nines/detail_page.asp?fileName=nl_1908_chicago.gif&Entryid=124

The Dallas Cowboys changed their unis before AND after their Super Bowl in '95:

1994: Wore their failed attempt at a 1960's Throwback for one monday night game vs Detroit:

...then replaced those with the infamous "Double Star" uniforms, which they wore for the rest of the season:

As mentioned in the other thread, the Devil Rays rebranded including a name change to the Rays after the 2007 season. The next year, the Rays completed a cinderella season for the ages by ending Boston and New York's stranglehold of the AL East and beating the Red Sox to claim the AL Pennant (Would've won the World Series if it wasn't for those Phillies  ).

From worst....

after the '94 season, They replaced their nice looking blue unis with a swapped version of the Double Star jerseys:

They wore these jerseys in only two games: Washington and Philadelphia. They lost both games. The Double Star unis were put to pasture, or at least until they wore them for a few years on Thanksgiving in the early 00's. Dallas then won Super Bowl XXX.

1996: Introduced a modern version of the Blue Unis, which they still wear to this day.

Patriots, changed to the current uniforms in 2000 and won 3 Super Bowls in 2001,03 and 04 seasons

That was sad. A beautiful old-school red, white, and blue look with Pat the Patriot replaced by a generic blue and silver brand with the Flying Elvis.

The change he was referring to was in 2000, not 1993.

IIRC, the Patriots changed their jerseys three straight years from 1993 to 95. Apex was their supplier the first two years (93-94, in which in 94 they went to sewn on numerals) and then changed to Starter in 95 when Apex went out of business. Might have been the only pro sports franchise ever to change looks three consecutive years. Obviously, the following year after that they won the AFC championship, and reached the Super Bowl.

I don't think the Raven's change counts because Nike's idea for the "toilet bowl" collars was a mistake to begin with and secondly, it wasn't a wholesale change
