As for the little Quidditch, book, it's called "Quidditch Through the Ages", and there is a list of teams in there, though my copy's in a box somewhere in the closet. EDIT: for a good look at some real professional uniforms, you can check out the Bulgarian and Irish uniforms in the beginning of the new Harry Potter film, which comes out in about a week.
